What is a Sleeve Gastrectomy?

Gastric Restrictive Weight Loss

Vertical Sleeve Gastrectomy (VSG), also known as gastric sleeve, is a surgical weight loss procedure that is performed with the goal of helping individuals achieve significant weight loss by causing chemical changes in your body and restricting the amount of food the stomach can hold. Gastric sleeve surgery involves the removal of a large portion of the stomach to create a smaller, sleeve-shaped stomach.

    How Sleeve Gastrectomy Works

The Science Behind Gastric Sleeve Surgery

    Stomach Reduction

During the surgery, a surgeon removes approximately 75-80% of the stomach, leaving behind a narrow tube or sleeve-like structure. The remaining stomach is shaped like a banana or sleeve.

    Restriction of Food Intake

The primary mechanism of weight loss with a gastric sleeve is restriction. With a smaller stomach, individuals experience a feeling of fullness with smaller amounts of food, which helps in reducing caloric intake.

    Hormonal Changes

Sleeve gastrectomy leads to changes in gut hormones such as ghrelin, by up to a 70% reduction, affecting hunger, satiety, and metabolic processes. This hormonal shift can contribute to the positive outcomes observed in weight loss and improvement in obesity-related health conditions.

Resolution of Comorbidites after Sleeve Gastrectomy

Gastric sleeve has been shown to resolve or improve:

    Type 2 Diabetes (45-58% Resolved)
    Sleep Apnea (60% Resolved)
    High Blood Pressure (50% Resolved)
    High Cholesterol (77% Resolved)

    Advantages of Vertical Sleeve Gastrectomy

Learn the Benefits of Gastric Sleeve Surgery

Gastric sleeve surgery offers several advantages for individuals seeking effective and lasting weight loss solutions. It’s important to note that while gastric sleeve surgery offers these advantages, it is not a one-size-fits-all solution. The decision to undergo any weight loss surgery should be made in consultation with healthcare professionals, considering individual health conditions, lifestyle factors, and personal goals.

    Significant Weight Loss

Gastric sleeve surgery is known to result in substantial weight loss. Patients often achieve a reduction in excess body weight ranging from 55% to 70% within the first year after the procedure.

    Reduced Appetite

The surgery removes a significant portion of the stomach, including the part responsible for producing the hunger-stimulating hormone ghrelin by up to 70%. This leads to a decreased appetite, making it easier for individuals to adhere to a reduced-calorie diet.

    Improved Obesity-Related Health Conditions

Many individuals experience improvement or resolution of obesity-related health conditions such as type 2 diabetes, high blood pressure, and sleep apnea after gastric sleeve surgery. This can contribute to an overall improvement in health and well-being.

    Minimally Invasive Procedure

Gastric sleeve surgery is typically performed using a laparoscopic approach, involving small incisions and a camera for visualization. This minimally invasive technique often results in a faster recovery, reduced pain, and a lower risk of complications compared to traditional open surgery.

    No Intestinal Bypass

Unlike gastric bypass surgery, gastric sleeve surgery does not involve rerouting or bypassing a segment of the small intestine. The digestive process remains more natural, reducing the risk of absorption-related complications.

    No Foreign Objects

Unlike gastric banding (lap band) procedures, gastric sleeve surgery does not involve the use of foreign objects. This eliminates the need for adjustments and reduces the risk of device-related complications.

    Preservation of the Pyloric Valve

The procedure typically preserves the pyloric valve, which regulates the release of stomach contents into the small intestine. This helps maintain a more normal digestive process.

    Faster Recovery Time.

Patients often experience a quicker recovery period with gastric sleeve surgery compared to some other weight loss procedures. Most individuals can return to normal activities within a few weeks

    Reduced Risk of Nutritional Deficiencies

Because the surgery does not involve rerouting the intestines, there is a lower risk of nutritional deficiencies compared to some other weight loss surgeries. However, patients are typically advised to take nutritional supplements to ensure adequate nutrient intake.

    Enhanced Quality of Life

Successful weight loss through gastric sleeve surgery can lead to an improved quality of life. Patients often report increased energy levels, improved mobility, and enhanced overall well-being.

      Frequently Asked Questions

Answering Your Common Questions About Sleeve Gastrectomy

Gastric sleeve surgery, also known as sleeve gastrectomy, involves the removal of a large portion of the stomach, creating a smaller, sleeve-shaped stomach. This restricts the amount of food the stomach can hold, leading to weight loss.

The ASMBS Guidelines now recommends bariatric surgery for individuals with a BMI of 35 or more regardless of presence, absence, or severity of obesity-related conditions and that it be considered for people with a BMI 30-34.9 with significant comorbidities such as Type 2 Diabetes, Obstructive Sleep Apnea, Uncontrollable Hypertension.  It is also recommended that obesity definitions using standard BMI thresholds be adjusted by population and that Asian individuals consider weight-loss surgery beginning at BMI 27.5.

Gastric sleeve surgery is a relatively quick procedure, often taking about under one hour.

Some patients are concerned about “irreversibility” of certain procedures, sleeve gastrectomy in particular. We used to get that question a lot when VSG (Vertical Sleeve Gastrectomy) first came out – more than 15 years ago!

The way it should be looked upon is the following.

      1. Obesity is a chronic, debilitating disease, and needs to be addressed permanently, not temporarily. Most of the patients who come to us for help, have the “obesity gene”, and therefore looking for a permanent solution, and have been struggling with weight problems their entire life.
      2. Having a “normal portion size” stomach capacity of 6-8 ounces after sleeve gastrectomy allows our patients to live a healthy, normal life, and there is no need to be concerned, therefore, with permanency of the procedure. It also allows for reduction of the hunger hormone, called ghrelin, by a factor of 70% in majority of the patients. Leaving a larger stomach may lead to weight loss plateau, possible weight regain.

All these discussions take place during the initial visit, and we make every effort to customize the treatment plan, based on the individual patients needs and goals. Please always ask our surgeons about different options available and which one is best for you.

Most individuals can return to normal activities within 1-2 weeks after surgery. However, strenuous activities may need to be avoided for a longer period.

Yes, adopting a healthier lifestyle is crucial for long-term success. This includes dietary modifications, regular exercise, and adherence to post-operative guidelines.

While complications are rare, they can include infection, bleeding, leakage, and adverse reactions to anesthesia. A thorough discussion of potential risks will occur during the pre-operative consultation. Our practice has a complication rate below the national average of .01% and recognized as a Center of Excellence in Bariatric Surgery by both the American College of Surgeons and the American Society for Metabolic and Bariatric Surgery (ASMBS).

Weight loss varies, but many individuals can expect to lose 55% to 70% within the first year after the procedure.

Since our nutrients are absorbed in the digestive system through the foods we consume, and the portions are reduced, nutritional supplementation  will be recommended early after surgery. Our healthcare team will provide guidance on necessary supplements to prevent nutritional deficiencies.

Yes, many individuals experience improvement or resolution of conditions such as type 2 diabetes, high blood pressure, and sleep apnea following gastric sleeve surgery. However, individual results may vary.
